improve apache log handling

apache logs should not be readable by "others", and should not be deleted after a week. All logs should use full format (combined, plus duration info).

Philipp also found some inconsistencies in the logging - different log files are using different log formats. It would also be nice to log request duration.

Any chance this might have been handled at some point? If not, we should look into getting it done :)
